Evolutioneyes: Mobile Optometry Services @ Bent Arrow

Evolutioneyes’ mobile optometry services will be on-site at Bent Arrow from July 21st through 25th!

On board of the mobile clinic, they offer complete eye exams, as well as glasses, sunglasses, contact lenses and safety glasses. If you register using the below link, the provider can pre-approve and direct bill for Treaty, Income Support, AISH, CFS, and company benefit plans.

👉 If you do not have coverage through one of these programs, you will be responsible for the cost, as this is a fee-for-service.

Booking Instructions:

1. Click HERE to book your on-site eye exam
2. Your name will appear as “Bent Arrow Society”
3. Click on “Complete Exam”, then click Next”
4. Select appointment time and enter your information

Reminders for Exam Day: 

1. Please bring the following documentation:
   1. Your Benefits Card (Band Info, AISH Info)
   2. ID
   3. Alberta Health Care Card / Number
2. If you wear contact lenses, please bring a pair to your exam, but you do not need to wear them (If you would like to renew your prescription, please also bring a copy of your previous lense prescription, or the bubble packs / boxes of the ones you are currently wearing)
3. Please bring your current glasses / contacts to the examination
